The medical field is getting advanced day by day, and we can see a lot of improvement in the technologies. But still, there is one unchanged thing, and that is the position which is used in the hospital beds. The position in the hospital bed is the most discussed topic in the health care sector.
In the terms of position, have you ever heard about the Trendelenburg position? This is the position given for the patients who are on the operating table and most common for the patients who have done the lower abdominal surgeries and central venous catheter placement.
The things to know about Trendelenburg Position:
The hospital bed adjustable position is the most important thing for the patients as it will ensure the safety of the patient who has undergone the surgical procedure. The position may vary for each patient depending on the type and length of surgical procedure, anesthesia access, and other such medical factors.
Most people don’t know what a Trendelenburg position is; this is the position where the patient will be laid supine on the surgical table and their head will be angled down. This Trendelenburg position will help the surgical team by giving them access to operate the patient’s abdominal organs. This kind of position will also help to move other organs in the abdominal cavity away from the surgical site.
Trendelenburg Position Vs Reverse Trendelenburg Position:
The modified version of the Trendelenburg position is the reverse Trendelenburg position. This is mainly used for laparoscopic surgeries that include the gall bladder, biliary tract, stomach procedures, and also head and neck surgeries.
When it comes to the Trendelenburg position, where the head is positioned down and the feet are positioned up. In reverse Trendelenburg position, the head will be positioned up, and the feet will be positioned down.
There are so many beds available for the reverse Trendelenburg position and the Trendelenburg bed will be titled where the feet will be facing downward, and the head is tilted to 15 degrees to 30 degrees. The head is supported by the headrest where the head will be in the neutral position so that the head and spine are aligned. Basic five positions of a patient in a hospital bed:
1. Fowler’s position
The Fowler’s position is also known as the sitting position, and it is mostly used for neurosurgery and shoulder surgeries. The patient’s head is placed at a 45-degree angle, and the hip may or may not be flexed. This is the most common position practiced in the medical field as it will offer more comfort and care to the patients. The Fowler’s position comes with a variation that includes low Fowler (15 to 30 degrees), semi Fowler (30 to 45 degrees), and high Fowler (nearly vertical). This kind of position is mainly for the patients who have difficulty in breathing and who have cardiac, respiratory, and neurological problems.
2. Supine position
Supine position is the most frequently used position in a medical procedure, and it is also known as Dorsal Decubitus. In this position, the patient will be faced upwards and the patient’s arms should be tucked at the sides. This kind of position is mainly used for the following procedures such as intracranial, cardiac, abdominal, endovascular, laparoscopic, lower extremity procedures, ENT, neck, and face.
Here, the patient will be lies flat on the back, and also there will be additional supportive devices that are added for comfort. But this position may risk pressure on the ulcers and nerve damage.
3. Prone position
In the prone position, the patient will be lies on the abdomen with the head turned to one side and the hips are not flexed. This is the only bed position that will allow full extension of hip and knee joints. The prone position is mostly used for neurosurgery and in most cases neck surgery and spine surgery. If the patient is placed in the prone position then the pressure should be kept off of the eyes, cheeks, ears, and breast. When the prone position is preferred for the patient, then at least there should be four surgical staff while turning the patient to the prone position.
4. Tripod position
The tripod or Orthopedic position is similar to the sitting position or the patient should be on the side of the bed with the over-bed table in front of the patient to lean, and several pillows will be placed on the table so that the patient can rest. This kind of position will offer a maximum expansion of the chest, so it would be suitable for the patients who are having breathing problems. Also, it will be helpful for the patients who are facing difficulty in exhaling because they can press the lower part of the chest against the edge of the over-bed table.
5. Lateral position
In the lateral or side-lying position, the patient will be lies on one side of the body and the top leg will be placed in front of the bottom leg, and the hip and knee will be flexed. This position will help the patients to relieve the stress on the coccyx. The risks include in the lateral position is experiencing the pressure on the dependent side of the body such as ears, shoulders, ribs, hips, knees, and ankles. If needed, we can place the pressure-reducing mattress or table-top.
